A Medicaid-approved program, Elant Choice is an innovative, community- based managed long-term care plan available in Orange and Rockland Counties that enables eligible adults, aged 18 or older, to receive care while remaining at home, in their community. Elant Choice enrollees have access to an affordable and coordinated system of care that provides much needed health care and social services in the most appropriate setting. Elant Choice provides the care and assistance needed to keep you or your loved one at home for as long as possible.
An alternative to residential nursing home care, Elant Choice fosters a sense of freedom, individuality, and personal dignity. Each client’s care plan is individualized to meet their medical, social, psychological, and environmental needs. Family and caregivers play an important role in developing the plan.

Trustworthy, compassionate healthcare paraprofessionals provide reliable and dignified care within the comfort of one's own home and familiar surroundings.
Assistance may be provided with some of life’s daily activities, including bathing, dressing, medication reminders, as well monitoring of blood pressure and other vital signs. The program is managed by an experienced registered nurse who may also assist with wound care, nutritional counseling, and medication management.

Long Term Home Health Care Elant’s Long Term Home Health Care programs provide comprehensive skilled nursing, therapy, personal care and social work services in the comfort and privacy of the client's own home. The program is open to anyone in Dutchess County eligible for care in a residential care facility or nursing home. The Long Term Home Health Care Program is covered by Medicaid, Medicare or private pay.
HIV Home Care Program A unique program in Dutchess County specifically for adults who are diagnosed with HIV-related illnesses. Similar to the Long Term Home Health Care program, the HIV Care program provides comprehensive care in the client’s home.
Adult Family Foster Care Elant’s unique adult family foster care program matches elderly or disabled adult clients with families who open their homes and their hearts to provide personal care for the client in a stimulating and nourishing family environment.
The program is ideal for elderly or disabled adults who do not need total care, but do require 24-hour supervision and cannot receive that care at home. For qualified clients, the Foster Family Care Program is a beneficial alternative to residential or nursing home care. Available exclusively in Dutchess County.
Foster families and their homes are carefully screened for suitability and family members receive extensive training before being accepted into the program. Compatibility between the client and Foster Family is paramount in making the appropriate match.
Caregivers receive payment for providing room and board and personal care services required by the client. Many Foster families enjoy the personal satisfaction of welcoming an older, uniquely experienced person into their family and their home. Elant’s Foster Family Care Program provides both clients and caregiver families with opportunities for richer, fuller lives.
