Long-Term Home Health Care Program
The only program of its kind in Dutchess County, the LTHHCP provides comprehensive care in the comfort and privacy of your own home. The program is open to anyone in the county eligible for care in a residential care facility or nursing home.
Admission is based on the result of a detailed assessment, after which a comprehensive care plan is developed by the primary nurse who coordinates the various services. The program is long-term in that it is designed to help clients stay in their homes for as long as possible.
The LTHHCP is covered by Medicaid, provided that the amount of service needed is within 75 percent of what it would cost to receive care in a local nursing home. Other sources of payment include private pay and Medicare for short-term care as applicable.
